PVFC Grievance Procedure:

Portneuf Valley Family Center, Inc. is committed to your satisfaction with services that you are receiving from our agency.  We understand that there may be things that you disagree with or feel has not been handled appropriately.  This form explains our grievance policy and process if you ever needed to express concerns or make a formal complaint.  Please make sure all information is completed accurately and completely.

If you have a concern or grievance with a staff member of Portneuf Valley Family Center, Inc. and have made every effort to resolve this grievance with this staff directly and have been unsatisfied you can submit your complaint in writing or verbally to a supervisor or administrator. 

You can do this by calling our administration office at (208) 233-7832 and request to speak to a person in management.  Please be prepared with accurate information regarding the situation, staff involved, date and time of the situation.  We take all consumer concerns seriously and will give the information the proper attention required to seek adequate resolution.  All information will be treated confidentially and will be shared with only those who are involved in the grievance procedure.  Once the information is gathered, we will investigate and make a determination as to the best course of action and provide you with the outcome and plan for resolution.
